Video Guide - Triage Process
Hello,
Dr Finucane here.
I am one of the GP partners at Ammonite Health Partnership, which covers Bridport Medical Centre, Pound Piece Surgery in Maiden Newton, and Tunnel Road Surgery in Beaminster.
The purpose of this video is to explain our Triage process and how best to access us. One of the key messages to take away from this short video is “help us to help you”.
Sickness Certificates (Fit Notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.
Travel Vaccinations
Information and advice for travelling abroad.
Home Visits
Please be aware that our GPs will always call to assess the home visit request first.
If you need a home visit, you can help us by calling reception before 11am.
Telephone: 01308 421 896
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby.
Change or Cancel an Appointment
Please give us as much notice as possible so we can offer your appointment to someone else.
- Please remember that there may be a shortage of appointments and when you fail to attend you have prevented someone else from being seen in your place.
- Cancellations notified less than 30 minutes before the appointment time will be recorded as failure to attend (DNA).
- Repeated failure to attend booked appointments is a significant waste of NHS resources.
Read our Did Not Attend Policy
