Our Services at Ammonite Health Partnership 


Annual Reviews

If you have a long-term health condition you will be invited in for your annual review during the month of your birthday.

However, in the meantime, if you are concerned about your health do not wait to be called for your annual review, either complete an eConsult form or call 111.


Asthma Management

At Ammonite, there are various medical services on offer for our patients with Asthma.

We offer annual Asthma reviews for our asthma patients to ensure their condition is optimally managed and their prescription remains safe.

When a patient is due their review, we will send them an Asthma questionnaire. Once completed our respiratory nurses will review and decide the best course of action.


Blood Pressure Check

If you would like to check your own blood pressure at any time, please use the machine located in the reception area. You can also buy Home Blood Pressure machines in some pharmacies and on Amazon.

Blood Tests

Blood tests have a wide range of uses and are one of the most common types of medical test. For example, a blood test can be used to:

  • Access your general state of health
  • Confirm the presence of a bacterial or viral infection
  • See how well certain organs, such as the liver and kidneys are functioning

Cervical Screening

The NHS invites patients eligible for a cervical screening text, if you have been invited for a cervical smear test with a nurse, please contact the surgery to book.

Child Health Assessment

Post natal and six week baby checks are done by appointment with your GP once the baby is registered at the practice. Other development arrangements are made with your health visitor.


Child Immunisations

Child immunisation clinics are held on Thursday mornings and Fridays for Bridport Medical Centre, and Thursdays for Tunnel Road surgery. If you require information regarding child immunisations, please speak to your health visitor or practice nurse


Chronic Disease Management

For:

  • Asthma 
  • Coronary heart disease (CHD)
  • Chronic obstruction pulmonary disease (COPD)
  • Diabetes
  • Stroke/TIA

Contraception and Sexual Health

COPD Management

At Ammonite, there are various medical services on offer for our patients with COPD.

We offer annual COPD reviews for our COPD patients to ensure their condition is optimally managed and their prescriptions remains safe. When a patient is due their review, we will send them an invitation to book an appointment with one of our respiratory nurses.


Diabetes Management

At Ammonite, there are various medical services on offer for our patients with diabetes.

We offer annual diabetes reviews for our diabetic patients to ensure their condition is optimally managed and their prescriptions remains safe. When a patient is due their review, we will send them an invitation to book an appointment with one of our nurses.


ECG

An ECG is a simple test that can be used to check your heart’s rhythm and electrical activity.

Sensors are attached to the skin to detect the electrical signals produced by your heart each time it beats, these signals are then recorded by a machine for your GP to look at.

If you have been requested to book an ECG, please contact the surgery to schedule an appointment with one of our Health Care Assistants.


Learning Disability Annual Health Check

It is important that everyone over the age of 14 with a Learning Disability has an Annual Health Check.

When a patient is due their review, we will send them an invitation to book an appointment with on one of our nurses.


Leg Clinic

Leg clinic appointments are available, Mondays, Tuesdays, Wednesdays and Fridays for Bridport Medical Centre.

Fridays for Pound Piece Surgery.


Minor Surgery

This will be arranged and carried out by prior arrangement with your GP

Social Prescribing

The Social Prescriber can assist with a range of issues, helping you identify what matters to you. This could include, assistance with health coaching, anxiety, isolation and loneliness, carer support and managing long term conditions. We can also connect you with local community activities and groups.

Test Results

When the clinician orders any tests you may require they will indicate whether you should make an appointment for the results, or whether you will be contacted via telephone if any further action is required.

Mental Health Services

Free talking therapies service for people aged 18 and over who are experiencing low mood and anxiety. Pick up a leaflet from reception. Patients can access this service via their GP, a nurse practitioner or other health professional. Patients aged 18 or over can refer themselves to the service.
